HALITOSE EM PORTADORES DE PRÓTESES SOBRE IMPLANTE
HALITOSIS IN PATIENTS WITH IMPLANT SUPPORTED PROSTHESIS

A halitose é uma condição multifatorial, cuja 90% das causas são intra-orais, principalmente devido à degradação da matéria orgânica por bactérias anaeróbias proteolíticas, produtoras de compostos sulfurados voláteis (CSVs). Objetivo: Avaliar a halitose em portadores de próteses sobre implante, identificar quais dessas próteses são mais diretamente associadas com o mau hálito e a influência da halitose na qualidade de vida. Materiais e métodos: Foram avaliados 84 pacientes, divididos em quatro grupos de 21 indivíduos: grupo 1, portadores de prótese total sobre implante do tipo protocolo; grupo 2, prótese parcial sobre implante; grupo 3, coroa unitária sobre implante e grupo controle, composto por pacientes que não usavam nenhum tipo de prótese. Os níveis dos CSVs foram medidos com o Halimeter®. No exame clínico foi avaliada a presença de cavidades de cárie, índice de placa, índice de sangramento gengival, nível clínico de inserção, profundidade de sondagem e índice de saburra lingual. Dois questionários foram usados para avaliar a qualidade de vida: OHIP-14 (Oral Health Impact Profile) e HALT (Halitosis Associated Life-Quality Test). Resultados: Foi observado que os grupo 1 e 2 tiveram os menores índices de CSVs (68 ppb e 63 ppb, respectivamente) em comparação aos do grupo 3 e 4 (187 ppb e 135,9 ppb, respectivamente). Foi feita uma análise ajustando o fator confundidor “periodontite” e nos pacientes sem periodontite, o nível de halitose no grupo 1 continuou significantemente mais baixo do que os grupos 3 e 4. Conclusão: A halitose foi menor nos pacientes com prótese total e parcial sobre implantes quando comparados aos pacientes com próteses unitárias e sem prótese, mesmo em pacientes sem periodontite. A qualidade de vida dos pacientes do estudo foi pouco afetada pela halitose e pelas doenças bucais.
