Ilheus virus (ILHV) was first identified in the city of Ilheus, Bahia state, Brazil. ILHV is an arbovirus belonging to the Flaviviridae genus, family Flaviviridae. The enzootic cycle between birds and mosquitoes maintains this virus in nature, with few reports of ILHV among humans. Objective: The aim of this work is to report an unusual and rare clinical presentation of HIV infection in humans. Methodology: Several laboratory investigations and molecular diagnosis of arboviruses are discussed. Results and Discussion: The atypical clinical presentation of ILHV infection in humans in Salvador, Bahia, Brazil is described, showing dermal necrosis associated with vasculitis, perivasculitis, and microthrombi in upper and lower limbs. Results showed that molecular diagnosis confirmed the presence of ILHV in serum. This report wants to reinforce the importance of epidemiological surveillance of emerging or re-emerging arboviruses and raise awareness among health professionals about atypical clinical presentations of ILHV infection.
