Abstract

Introduction: Supernumerary teeth represent a developmental anomaly resulting in additional teeth beyond the typical sequence, often in anomalous locations. Osteoradionecrosis (ORN) is a complication derived from radiotherapy (RT), affecting bone metabolism and preventing its normal regeneration. Objective: To report the endodontic treatment of a supernumerary tooth in a patient with osteoradionecrosis (ORN) after head and neck radiotherapy. Methodology: the methodology adopted in this article was a case report, through the care of a 39-year-old female patient who attended the clinic at the State University of Feira de Santana complaining of pain and swelling in the region of her upper right molar. Clinical examination revealed a supernumerary tooth with extensive decay. Furthermore, ORN was diagnosed in the left upper molar region. Radiographic and endodontic evaluation indicated pulp necrosis associated with chronic apical periodontitis. Results: endodontic treatment of the supernumerary was carried out in a minimally invasive way due to ORN, aiming to eliminate the infectious focus and minimize the risks associated with the patient's condition. The evaluation carried out six months after treatment showed the absence of clinical symptoms and the beginning of bone repair in the region. Conclusion: This case report highlights the challenges and importance of endodontic treatment in patients undergoing radiotherapy, especially in teeth with anatomical variations, to maintain the tooth and avoid the risk of osteoradionecrosis.
